I believe the answer should be:

var id = "23188xtr";
var lastName = "Smith";

var userName = id.toUpperCase() + '#' + lastName.toUpperCase();

Since you can't pass more than one parameter in this method (Mostly due to the fact that it can only be used with one variable, ex: ID.toUpperCase(), LASTNAME.toUpperCase) so adding them together and concatenating the hash sign ("#") between them should give you the answer you're looking for. Also, you don't need quotation marks for variable names, otherwise, Javascript would read them as strings. :D
