var userName = id.toUpperCase() + "#" + lastName.toUpperCase();

The problem here is a common one: a syntax error! You've got a semi-colon still sitting after the initial assignment to the userName variable, i.e. the 'id.toUpperCase();' portion. Remove the semi-colon after 'id.toUpperCase()' and you're good to go!
