You question is a little to complex to try to answer fully in a forum post. In general though, you'll need a little bit of html knowledge, and of course php. You can look at the bootstrap documentation to see how to set up a form using the bootstrap classes and markup, and than you'll use php to parse the form submission into something you can do with it, whether that be save it to a database, or send an email - whatever you want.
If you get something going, I'd be happy to try to help you with specific problems you are having. Otherwise, I recommend going through the php lessons here, it'll show you how to build a form, and get the submitted form details.
