How can one include an image tag without adding any alt attributes or caption?

Unable to identify the associated bug.

So what it's asking for is a blank alt attribute. You can leave the captions completely gone. An empty alt attribute looks like this:

Sorry I didn't look closely at your code, here is what mine looked like to pass:
