How did the author create the database.db file?

Hello there! I just finished the chapter, Querying the Database with PHP, in the Integrating PHP with Databases course. But I am confused. The author showed how to perform the query, but how did she created the database.db file and the media table? How many columns and rows she created? She didn't cover it, why?
