i don't understand when it says: Complete the assignment to the userName variable by adding a # symbol followed by an all uppercase version of the lastName variable.

It wants you to store "23188XTR#SMITH" in the username variable. You're already given id ("23188xtr") and lastName ("Smith"). So, all you have to do is concatenate the two variables with a "#" sign in between, like this:
var userName = id.toUpperCase() + '#' + lastName.toUpperCase();
