PHP Build a Simple PHP Application Adding a Contact Form Working with Get Variables

lost my form after I put in the GET function.

The contact form is gone after I completed the GET variable lesson and my form no longer shows on the web page. Not sure what I did wrong.

Do you seen anything on the page? If you see nothing, there's likely to be a syntax error in any part of the page. I would enter the following code at different points throughout the page to find where it's breaking. 

echo "test";
exit;

If the test message isn't showing up, even at the very top of the file, you can look for things like - including files that don't exist, or calling functions that don't exist/with invalid arguments.
