As Jacob said above, make sure there is never space between your opening or closing PHP tags. Also, this applies to a lot of coding, spaces and unwanted characters can make all of you're code malfunction.

Correct

<?php
  $name = "Mike";
?>

Not Correct

<? php
  $ name = "Mike";
?>

You have a space in your opening PHP tag.

Correct:

<?php
  $name = "Mike";
?>
