Some issues with the php shirt store

I have been following the Shirts 4 Mike section, and after unpacking project files etc, and following the lessons, I have the following errors when I view the site in my browser;

Dean, have you added code to the project? Or are you seeing this issue right after opening the files?

Hi Eric. I upacked the files, and have them in the htdocs folder as needed. Ive worked up to 'Adding Active States to The Navigations' in the second module of the 'Simple PHP Application'. Im almost certain its something Ive doen, but Im not entirely sure what, as I have followed the lessons and even compared my code to the code in the videos.
