The # needs to be treated as a separate string. Currently you have it attached to last.toUpperCase().You are also incorrectly re-declaring the userName variable.

Try this:

var id = "23188xtr";
var lastName = "Smith";

var userName = id.toUpperCase() + "#" + lastName.toUpperCase();
