Try this:

    form = forms.LeadShareForm()
    if request.method == 'POST':
        form = forms.LeadShareForm(request.POST) # this line is what you forgot
        if form.is_valid():
            return HttpResponseRedirect(reverse("lead")) 
    return render(request, 'lead_form.html', {'form': form})
