Hi, question 3 is asking for a label like this:

  <label for="color">Shirt Color</label>
  <select id="color" name="shirt_color">

Use a label tag and remove optgroup tag and it will be fine :)

It wants a label not a labelled option group:

<!DOCTYPE html>
<html>
  <head>
    <meta charset="utf-8">
    <title>HTML Forms</title>
  </head>
  <body>

    <form action="index.html" method="post">
      <h1>Shirt Order Form</h1>
      <label for="color">Shirt Color</label>
      <select id="color" name="shirt_color">
            <option value="red">Red</option>
            <option  value="yellow">Yellow</option>
            <option value="purple">Purple</option>
            <option value="blue">Blue</option>
            <option value="green">Green</option>
            <option value="orange">Orange</option>
      </select>
    </form>
  </body>
