- Full name should be replaced with separate fields for first and last name. You cant count on people writing it in the same order.
- I don't personally like how the field's form changes when selected. I'd keep it rounded when selected.
- What if your user is not from the States?
- 'Required' and 'Choose state' should be aligned on the same side

Nicely done, first I should suggest to make the radio buttons work. If I where you I should start with a simple site. For forums you need to code on the back-end to do something with it. So in my opinion you are a step to far. I'm curious about how you site wil look like, let me know.
