Don't have too much experience but what i have noticed so far is that you did not closed your tags "form" and "textarea", for example look at the end

Alex Zoidis Here are a few things

  1. Input tag is self closing have a <input .... />
  2. text area needs a separate closing tag <textarea></textarea>
  3. close the form tag <form ...> .... </form>
